The Form 1095-C, which contains information about your employer-provided health coverage, is now available upon request. While not required to file your tax return, this document can be used to verify your coverage.
To request a copy, please contact the United Therapeutics HR- Benefits Department via email at HRWorkday@unither.com, by phone at (301) 608-9292 or by mailing a request to United Therapeutics ATTN: HR Benefits 1000 Spring Street, Silver Spring, MD 20910. Employees who work in California, District of Columbia, New Jersey, and Rhode Island will receive a paper copy of their 1095-C in compliance with state/local guidelines.
Requests will be fulfilled within 30 days.
